Payment of Fees
A ssessed fees, as part of an individual’s sanctions to successful completion, are due upon the first scheduled appointment. Payment will be entered and tracked through the program provider’s cash management system and provide the individual with a receipt of transaction.Should an individual not have the means to make a payment in full, the provider may establish a payment plan that both the individual and provider agree to.
Should an individual claim indigence or the inability to pay, the individual may choose to complete a Financial & Fee Waiver Request form to be reviewed by a Review Committee.
